If you have health insurance, it almost certainly covers addiction treatment. The Affordable Care Act made substance use disorder treatment an essential health benefit, and the federal parity law requires plans to cover behavioral health on terms comparable to medical and surgical care. Insurers can't legally treat rehab as an optional extra.

The five words that decide your bill

Deductible

What you pay before the plan starts sharing costs. Resets each plan year, starting treatment in November vs. January changes your math substantially.

Coinsurance / copay

Your share after the deductible. Coinsurance is a percentage (often 10–30%); a copay is a flat per-visit amount.

Out-of-pocket maximum

The ceiling. Once you hit it, covered in-network care is paid at 100% for the rest of the year, which is why a full course of treatment often costs less than people fear.

Prior authorization

Medical necessity

Level of care changes the cost more than anything

Inpatient and residential care is the expensive end, daily room, board, and staffing. Outpatient is far less, and it's also what most people actually need. A structured intensive outpatient program three evenings a week costs a fraction of residential treatment and, for people with stable housing and support at home, produces comparable outcomes. What about "free rehab" in Maine?

People search for free rehab because they're out of options, and they deserve a straight answer. Truly free options in Maine do exist, but they're specific:

  • MaineCare (Medicaid) covers outpatient treatment and MAT with little or no cost-sharing. Maine expanded eligibility, so a lot of working adults qualify who assume they don't.
  • State-funded slots through Maine's SAMHS system serve uninsured residents; availability varies and waits are real.

What isn't accurate is the ad-driven promise of free luxury rehab. For most insured people in Southern Maine, the honest version is: outpatient treatment is covered, your cost is a deductible and a modest share after it, and it ends when your out-of-pocket maximum is reached.

What will I actually pay out of pocket?

It depends on three numbers: your remaining deductible, your coinsurance or copay, and your out-of-pocket maximum. Someone mid-year who's already met a deductible may pay very little for IOP. Someone starting in January may pay toward the deductible first. Once the out-of-pocket max is hit, covered care is fully paid for the rest of the plan year.

What if my plan says the provider is out of network?

Out-of-network doesn't mean no coverage. Many PPO plans still pay a percentage, single-case agreements are sometimes possible, and some plans must cover out-of-network care when no in-network option is available within a reasonable distance. Ask specifically about network adequacy rather than accepting the first no.
