Using Cigna insurance for addiction treatment in Maine

Out-of-network provider

Cigna is the one major payer Portland Treatment is not contracted with. We bill as an out-of-network provider and pursue single-case agreements where possible.

Straight answer first: Cigna is the only major payer Portland Treatment is not in-network with. We are in-network with Anthem, UMR/UnitedHealthcare, Aetna, Harvard Pilgrim, Martin's Point and MaineCare, but Cigna care is billed as an out-of-network benefit. That does not mean you have no coverage. Many Cigna and Evernorth plans still pay a meaningful share of out-of-network outpatient treatment, and single-case agreements are sometimes available. Here's how it works, and what determines your cost.

What's covered

What Cigna typically covers at Portland Treatment

Cigna treats substance use disorder as a medical condition, and parity law requires comparable coverage to physical health care. We bill these services to Cigna as an out-of-network provider, and we tell you the expected cost in writing before you start:

Partial Hospitalization (PHP)

Our most structured outpatient level, full days of clinical programming. Cigna plans typically cover PHP with prior authorization, which our admissions team requests for you.

More about partial hospitalization (php)

Intensive Outpatient (IOP)

Three to five sessions a week, mornings or evenings, so you can keep working. IOP is the most commonly authorized level of care under Cigna behavioral health benefits.

Outpatient Program (OP)

Weekly individual and group therapy for step-down and long-term maintenance. Usually the lowest-cost level under a Cigna plan.

More about outpatient program (op)

Medication-Assisted Treatment (MAT)

Buprenorphine, naltrexone and related medications, billed through your medical and pharmacy benefits. Coverage and preferred formulary vary by Cigna plan.

Dual diagnosis care

Depression, anxiety, PTSD and bipolar disorder treated alongside substance use, one treatment plan, one set of clinicians, billed under your behavioral health benefit.

Detox placement

We don't run a detox unit. We coordinate placement with licensed Maine detox providers and confirm they take your Cigna plan before you go.

Know which Cigna plan you actually have

Which Cigna product you have shapes networks, referrals and how much of the cost lands on you.

Open Access Plus (OAP)

Cigna's most common employer PPO-style plan and the best case for us. No referrals needed, and out-of-network outpatient benefits usually pay a real percentage of the cost.

LocalPlus and HMO plans

Narrow networks with little or no out-of-network coverage. On these plans we look at a single-case agreement or a network-adequacy exception before you commit to anything.

Cigna + Oscar and marketplace plans

Individual coverage with substance use treatment as an essential health benefit, often paired with a high deductible.

Evernorth behavioral carve-outs

Most Cigna behavioral health claims route through Evernorth, with its own authorization process for intensive levels of care.

What you'll pay

What determines your out-of-pocket cost

Because we're out-of-network with Cigna, cost depends heavily on whether your plan has out-of-network outpatient benefits. These are the factors:

  • Whether your Cigna plan includes out-of-network outpatient behavioral health benefits at all, PPO and OAP plans usually do, HMO and LocalPlus plans often do not.
  • Your separate out-of-network deductible and coinsurance percentage, which are typically higher than the in-network figures printed first on your card.
  • Whether Evernorth will approve a single-case agreement, which pays us at an in-network-style rate for your episode of care.
  • Whether network adequacy applies, if no comparable in-network program is available within a reasonable distance, plans are sometimes required to cover us at the in-network level.
  • Your out-of-network out-of-pocket maximum, and any self-pay arrangement we set up if using benefits doesn't make financial sense for you.

How verification works

  1. 1. You call or submit. We take your Cigna member ID and date of birth, nothing more invasive than that.
  2. 2. We call the plan. Our team gets the behavioral health benefit read directly from Cigna Healthcare / Evernorth Behavioral Health, in writing.
  3. 3. You get a real number. Deductible remaining, coinsurance, authorization requirements and an estimated cost per week, before you commit.
  4. 4. We handle the rest. Prior authorizations, concurrent reviews and appeals are our job, not yours.

Southern Maine

Using Cigna coverage close to home

Cigna is common among Maine employees of national companies, and we regularly work with clients commuting to our Biddeford campus from Portland, South Portland, Scarborough, Saco and Kennebunk. Outpatient care close to home is also easier to authorize than distant residential placement, which works in your favor clinically and financially.

Our admissions team calls Evernorth directly, documents your out-of-network benefit in writing, requests a single-case agreement or network-adequacy exception where it applies, and gives you a real cost estimate before your first session. If a Cigna plan simply doesn't work financially, we'll say so and help you find an in-network option rather than quietly enrolling you.

Cigna coverage questions

Is Portland Treatment in-network with Cigna?
No. Cigna is the one major payer we are not contracted with. We are in-network with Anthem, UMR/UnitedHealthcare, Aetna, Harvard Pilgrim, Martin's Point and MaineCare. Cigna members are billed as out-of-network, and many plans still cover a share of the cost.
Does Cigna cover rehab?
Yes, Cigna covers substance use disorder treatment including PHP, IOP, outpatient therapy and medication-assisted treatment through Evernorth Behavioral Health. With us, that coverage is paid under your out-of-network outpatient benefit unless a single-case agreement is approved.
What is Evernorth and why is it on my Cigna card?
Evernorth is Cigna's behavioral health division. Mental health and substance use claims and authorizations typically go through Evernorth even though your card says Cigna.
Does Cigna require prior authorization for IOP or PHP?
Usually yes for PHP, and often for IOP. We complete the assessment and submit the request, then manage concurrent reviews so treatment isn't interrupted.
Does Cigna cover Suboxone and other MAT medications?
Yes, subject to formulary rules and sometimes prior authorization. We verify your pharmacy benefit alongside your behavioral health benefit.
How quickly can I start treatment with Cigna coverage?
Most Cigna verifications come back the same day, and admission usually follows within a few days, often within twenty-four hours when clinical urgency warrants it.
Is Cigna telehealth addiction treatment covered in Maine?
Cigna covers virtual behavioral health broadly, including many outpatient and IOP services delivered by telehealth. We confirm your specific plan's telehealth terms during verification.
