What is medically supervised detox?

A structured, monitored withdrawal, not "toughing it out"

Medical detox, sometimes called medically managed withdrawal, is a structured process in which you're monitored by medical and clinical staff while your body clears a substance. What that monitoring looks like, and how much risk is involved, depends heavily on what's being used:

Alcohol & benzodiazepines

Withdrawal from these can become medically dangerous, including seizures and delirium tremens (DTs) that typically begin 48–72 hours after the last drink. This combination is the most likely to require inpatient medical detox, in a medical setting these complications are managed and largely preventable; at home, they're a 911 call.

Opioids (fentanyl, heroin, painkillers)

Rarely life-threatening on its own, but intensely uncomfortable, nausea, vomiting, muscle aches, sweating, insomnia. Medical support makes this phase more tolerable and lowers the chance of returning to use just to stop the discomfort, often at a lower tolerance, which is when overdoses happen.

Stimulants (meth, cocaine)

Mostly physical and mental fatigue rather than acute medical danger, exhaustion, low mood, increased appetite, strong cravings. Supervision still matters, because the crash is when depression and relapse peak.

Who actually needs detox?

Inpatient medical detox

24/7 medical supervision in a licensed facility. The right call for alcohol and benzodiazepine withdrawal, polysubstance use, a history of seizures or complicated withdrawals, or significant medical and mental-health conditions.

Outpatient withdrawal monitoring

For some people, milder dependence, stable health, a safe and supportive home, withdrawal can be managed with scheduled medical check-ins rather than a residential stay.

Different substances, different detox plans

Alcohol

Typically 4–7 days

Withdrawal peaks around days 2–3. Medical teams use tapers and monitoring to prevent seizures and DTs. The most dangerous detox to attempt alone, and the one we get asked about most.

Fentanyl & opioids

Typically 4–7 days

Symptoms feel like the worst flu of your life, plus cravings. Medication-assisted starts (buprenorphine) in detox make completion far more likely and set you up for MAT in our outpatient programs.

Benzodiazepines

Often 1–2+ weeks

Benzo tapers must be gradual. A proper medical detox manages the taper schedule, watches for seizure risk, and treats the anxiety rebound that makes quitting so hard.

Stimulants (meth, cocaine)

Typically 3–5 days

Stimulant withdrawal is less physically dangerous but brings crushing fatigue, depression, and sleep disruption. Supervision matters because the crash is when suicidal thinking and relapse peak.

No, and we think it's important to be honest about that. Detox requires 24/7 medical supervision that outpatient programs aren't licensed to provide. What we do is help you find the right licensed detox provider for your specific situation, coordinate your admission, and then reserve your place in our PHP, IOP, or outpatient program so you step directly into treatment the day you're discharged.

Do I actually need medical detox, or can I start treatment right away?

It depends on the substance, how much and how long you've been using, and your health history. Alcohol and benzodiazepine withdrawal often require inpatient medical detox; opioid withdrawal usually benefits from medical support; some people with milder dependence can begin PHP or IOP without a separate detox step. A proper medical assessment, not a generic checklist, is what makes that call, and it's part of your first conversation with us.

Is it dangerous to detox from alcohol at home?

Yes. Alcohol withdrawal can cause seizures and delirium tremens (DTs), which can be life-threatening without medical care.
